Privacy Notice & Consent to Share Information

The 815 Senior Care Directory (“the Directory,” “we,” “us”) is a local directory operated by Certainty Publishing LLC that helps families in Winnebago and Boone counties, Illinois, connect with senior care providers and related professionals.

2. What Information We Collect

When you use our “Get help finding care” or “Request info” form, we collect the information you provide, which may include:

• Your name and contact details (phone, email);
• The type of care you are seeking (for example, assisted living, in-home care, memory care);
• The location or service area where care is needed;
• Your desired timeframe; and
• Your budget or payment source, including whether Medicaid or other public benefits may apply.

You choose what to share. You are not required to provide sensitive details to browse the Directory; this information is used to match you with appropriate providers.

3. How We Use Your Information

We use the information you submit to:

• Match you with senior care providers and professionals that fit your stated needs;
• Share your request with those matched providers so they can contact you directly; and
• Improve the Directory and follow up with you about your request.

4. Who We Share It With

By submitting the form, you understand and agree that we will share the information you provide with the care providers we match to your request, so that they can contact you. These providers are independent businesses, not part of the Directory. Once your information is shared with a provider, that provider's own privacy practices govern how they use it.

Paid placement disclosure: Some providers pay to appear as “Featured” or “Sponsored” listings. Paid placement does not mean a provider is endorsed or is the best fit for you; it means they have purchased visibility. We disclose paid placements on the site.

5. Your Consent

By checking the consent box and submitting the form, you confirm that you have read this notice and consent to our sharing the information you provided with matched care providers for the purpose of helping you find care. You may decline to submit the form and instead contact providers directly using the phone numbers listed in the Directory.

6. Data Security & Retention

We take reasonable measures to protect the information you submit, including restricting access so that a provider sees only the requests routed to them. We retain lead information for 24 months and then delete or de-identify it, except where a longer period is required.

7. Your Choices

You may request that we correct or delete your information, or stop sharing it with additional providers, by contacting us at [email protected]. We will honor these requests to the extent we are able, recognizing that providers already contacted control their own records.

8. Children

The Directory is intended for adults seeking care for themselves or their family members. It is not directed to children.

9. Changes to This Notice

We may update this notice from time to time. The current version will always be posted on the Directory, with its effective date.
